Geology Museum Field Trips

Geology Museum - Field Trip Registration

Please fill out the following form to register for a Field Trip. Once we receive your completed registration form, we will send you notification by email along with the total amount due to finalize your registration. After we receive your full payment, signed waivers, and signed consent forms you will receive final confirmation of your field trip spots as well as important information you will need for the day of the trip.

Each participant will need to fill out and sign an Informed Consent form. Additionally, all minors attending the field trip must have the following form signed by their legal guardian. Both forms will be provided upon registration and must be returned to the Geology Museum prior to the day of the field trip.

No refunds will be granted after field trip confirmation is sent. Field trip date transfers will be granted to those who notify the museum of any changes a minimum of 2 days prior to the scheduled trip. No refunds or trip transfers will be granted to no shows.
